Cesarean (c-section) for twins
Cesarean delivery (c-section) is defined as the delivery of a fetus through surgical incisions made through the abdominal wall and the uterine wall. This is the moment when the mother and the family see their babies for the first time. All arrangements should be made so that this special moment is safe both for the mother and the babies, and they receive the care of the best healthcare professionals.
Cesareans are surgical procedures and can be categorized as primary – when it’s the first c-section – or repeated – when the c-section follows a previous one.
The term “multiple birth” refers to the birth of twins – two or more babies – and happens when several fetus have origin in the same pregnancy. Twins can be categorized as monozygotic or dizygotic. The monozygotic twins occur when a single fertilized egg splits into two after the conception. The incidence of monozygotic twins is constant in the world, with approximately four for every 1000 births. Dizygotic twins result from the fertilization and development of two different and independent eggs.
The decision to do a cesarean delivery (c-section) is performed for maternal indications, fetal indications, or both. The greatest indicators of a cesarean are a previous cesarean delivery and the fetus position that might indicate the difficulty of a natural birth. When the babies aren’t in an appropriate position for a natural birth or when complications are likely to occur, a twin cesarean is the choice. A twin cesarean is similar to a single fetus c-section, since there is no known risk for the mother or the fetus. Continuous care during a twin pregnancy is important because it allows predicting more precisely the date of birth and the most adequate way for the delivery.
In a surgical procedure, the administration of anesthetics is essential. The pre-anesthetic evaluation should take into account the wellbeing of both patients: the mother and the babies, the physiological changes of pregnancy and the fetuses health status. This pre-evaluation is similar for other preoperative patients, being advisable a pre-consultation with an anesthesiologist. The experience of labor is remarkable for a woman. Bonding and trust between obstetrics healthcare professionals, the pregnant woman and her family are fundamental. The mother-to-be should be able to concentrate in the babies and trust those professionals around her in every stage of the labor, as well as before and after it.

The major goal of prenatal care is to ensure the birth of healthy babies with minimal risk for the mother. This is especially important in a twin pregnancy, extra care should be planned in order to ensure the pregnancy evolves as expected and no complications arise for the mother and the babies.
There are several components included in this goal:
- Accurate estimation of the gestational age at the earliest time
- Identification of possible risks for the mother and the babies
- Ongoing evaluation of the heath status of both
- Anticipation of problems and intervention if it’s needed
A pregnant woman may travel by plane on the first and second trimesters without restrictions, when there isn’t a high-risk pregnancy.
In order to travel by plane in the third trimester it is necessary to have a medical certificate, stating the health status of the mother and the babies as well as the predicted due date. When there’s no high risk involved in the twin pregnancy, the woman can travel by plane up to the 32nd week of twin pregnancy, or later if needed and approved by a doctor and the airline. It is not allowed to travel the 10 days proceeding the due date.
Although it is advisable to wait for the babies to complete 1 month before travel, newborns and their mothers can travel by plane 7 days after the birth, when no complications occur.
